What is Leptomeningeal Metastases?

The leptomeninges are the membranes lining the spinal cord and brain. The inner two meninges, which doctors refer to as arachnoid and the pia mater, circulates the cerebrospinal fluid (CSF). Cancer in this location has different names, including leptomeningeal metastases (LM), carcinomatous meningitis, and neoplastic meningitis. Cancer cells may metastasize (spread) from other parts of the body through the blood vessels, or directly from the bones of the spine.

Leptomeningeal metastases are uncommon, but they are serious complications of cancer, including melanoma, breast cancer, and lung cancer. In the United States, as little as 1 to as much as 8 percent of cancer patients are diagnosed with leptomeningeal disease each year, with approximately 110,000 cases per year.

Symptoms of Leptomeningeal Metastases

Signs of leptomeningeal metastases result from cancer cells blocking the normal exits for CSF, which causes fluid buildup and increased pressure in the brain. As such, someone with LM may have one or more of these symptoms:

  • Persistent headaches
  • Vision changes
  • Nausea
  • Difficulty speaking or swallowing
  • Difficulty thinking
  • Lack of coordination in your arms and legs
  • Seizures
  • Loss of bladder or bowel control

LM can also disturb nerves exiting the brain, causing:

  • Pain
  • Numbness
  • Weakness
